Each case management team has a nurse assigned. The nurse completes a periodic health assessment with all clients.
Nursing staff are available to assist clients with ongoing medical concerns.
When needed, the nursing staff are able to coordinate medical care with the agency’s Primary Care Clinic and other healthcare specialists.
When a client is admitted to the hospital for medical and/or psychiatric reasons, the client is scheduled for a follow-up appointment with the nurse following the discharge. During this appointment, the nurse will review medications and discuss any ongoing concerns.
Lab Services
Blood work is an essential part of diagnosis and treatment of mental and physical health conditions. Community Support Services offers on-site lab services through Pathology Laboratories, for our clients’ convenience.
Lab results are provided to the requesting physician with copies available for other medical providers, as requested.
