Regional Case Management

The majority of clients receiving services from Community Support Services are assigned to one of four regional case management teams. Assignment is based on the individual’s ZIP code.

A treatment plan is developed with the individual identifying his or her strengths, needs, abilities, and preferences.

This plan helps to guide the services he or she will receive. Services are based on the individuals’ identified levels of care. Our case managers assist clients in numerous ways with an emphasis on self-efficacy and self-sufficiency.

The case manager focuses on skill-building activities to assist the clients to accomplish goals.

Services are delivered in the community – clients’ homes, community sites, jail, hospitals, etc. The case manager is a partner in the recovery process. The case manager is able to connect the client to agency resources and community supports based on the identified needs.
